Watermark 4B Spiritual Growth Assessment
Watermark has set up a spiritual transformation process for all members. The first step is completing a 4B Spiritual Growth Assessment, formerly called the 5C Form, (click here to get started). The Assessment will help you see where you need to become better equipped as you seek to:
BELIEVE in Christ
BELONG to His Body
BE TRAINED in Truth
BE STRONG in Ministry
The 4B Spiritual Growth Assessment is required annually for all members in order for your membership to remain active. The deadline for completing your Assessment is February 14.
The 4B Spiritual Growth Assessment is the first step in a larger Spiritual Transformation Process:

- Take the 4B: Spiritual Growth Assessment available at
- survey.watermark.org.
- Review your results and pray for God to transform you.
- Share the results with those close to you (ie: your community group) and discuss the opportunities for growth
- they see.
PLAN
- Review the ‘Next Steps’ partof the Assessment —
- recommended tools/ resources to help you make progress in key areas of your spiritual life.
- Choose a few of the recommended classes, tools and resources.
- Ask for accountability as you move forward.
ACT
- Act on your spiritual growth plan. Consistency is key.
- Keep your group posted onyour progress.
- Regularly review your plan (on your own and in community)
- and evaluate your progress.
